Rachel Xu, MD, CLCP

Physician Life Care Planner Expert

Dr. Xu is a double board-certified Physical Medicine & Rehabilitation (Physiatry) and Pain Medicine physician who provides expert witness services and comprehensive life care planning for personal injury and litigation matters. As a physiatrist and interventional pain specialist, she evaluates the whole patient across the full spectrum of physiatric care—musculoskeletal, spinal, and neurological conditions, post-surgical and post-traumatic recovery, and acute and chronic pain—addressing function, mobility, and long-term rehabilitation needs. 

Dr. Xu completed her Pain Medicine Fellowship at UT Health San Antonio, where she was honored as Fellow of the Year, following her Physical Medicine and Rehabilitation residency at the Icahn School of Medicine at Mount Sinai in New York City. In private interventional pain practice, she performs the full range of fluoroscopically guided spinal and musculoskeletal procedures in accordance with evidence-based guidelines. She previously served as an attending physician at the South Texas Veterans Health Care System, where she practiced general physiatry and interventional pain management—evaluating and treating veterans with a broad range of musculoskeletal, spinal, neurological, and chronic pain conditions.

Dr. Xu maintains academic and research affiliations with UT Health San Antonio, reflecting her ongoing commitment to medical education, mentorship, and advancing evidence-based patient care.  She is also the co-author of the textbooks Musculoskeletal Pain Conditions: A Comprehensive Guide (Springer) and Fluoro-Flip: A Quick Reference Guide to Spinal and Peripheral Pain Procedures (Jaypee Brothers).

Before entering medicine, Dr. Xu earned a Bachelor of Science in Biomedical Engineering from Johns Hopkins University, providing a strong foundation in analytical reasoning and critical evaluation of complex data. She applies a structured, methodology-driven approach to case review, integrating current medical literature, clinical experience, and objective analysis to provide balanced opinions.

As a Certified Life Care Planner, Dr. Xu develops future care plans that are medically grounded, evidence-based, and consistent with accepted standards of clinical practice and life care planning.
